As with scaffolds, the balance beam has strict design standards; unfortunately this complies with neither set of standards.
If a gymnast falls from the balance beam they automatically incur a deduction. At the elite/world-class level this penalty is 0.8. If you fell from this scaffold, the deduction could be the rest of your life.

These workers could unintentionally be set to perform a reverse one and a half somersault off this scaffold, which would be a high scoring dive for style, but probably fatal.
Where to start:
- No fall protection (handrails & midrails) on the working platforms
- Working platforms not completely decked out (missing scaffold planks)
- Lack of ties on the scaffold to provide rigidity
- Lack of safe access to working platforms and lack of toeboards

The workplace safety inspector who submitted these images, also told a frightening tale.
"To be correct in its identification the OH&S Regulations call it an Industrial Lift Truck, but we know them simply as forklifts."
"But; what ever you call it, don't call it an over sized Molotov cocktail."
"The use of this one was prohibited firstly as a fire/explosion risk and secondly as the operator could not produce Certification/Licence."
"A fuel cap was found within the hour..." |
